Seine-et-Marne: large patient lists across the biggest département in Île-de-France
Seine-et-Marne alone covers close to half of the region's land area, taking in the new towns of Marne-la-Vallée, the Melun and Meaux urban areas, the Fontainebleau forest and the farmland of the Brie plateau. Across that ground, 611 general dentists work in private practice, 41 per 100,000 residents against 55 nationally. Only Seine-Saint-Denis and Essonne are less densely served in the region. The result is a broad caseload: each dentist sees 931 different patients a year, well above the French average of 837.
Fees follow that demand. A full-time active general dentist bills €374,516 on average, +24 % on France and +25 % on Île-de-France, which places the département at rank 15 of 101; within the region only the Val-d'Oise is higher. The amount divides almost equally between regulated tariff work (€187,417) and dépassements (€187,099), the extra fees charged above the agreed tariff, mostly on prosthetics.
Fees have climbed fast since 2019
The local average was €280,461 in 2015 and €303,367 in 2019. Most of the growth came afterwards, +23 % in six years compared with +17 % for France. Headcount barely shifted over the same stretch, from 589 at the end of 2020 to 611 at the end of 2025. So the rise does not come from fewer dentists sharing the work; each practice simply bills more.
The profession is fairly young and mostly self-employed. 32 % of general dentists are under 35 and 20 % are 60 or older, in line with the national 19 %, while mixed practice (private work alongside a salaried post) concerns only 9 %. Women make up 44 % of the workforce, a little under the country-wide share. For a practice hiring an associate or a dental assistant, these long patient lists mean full diaries; the guide to dental assistant pay sets out the cost side.
Few orthodontists, high averages
There are 35 orthodontists (specialists in orthopédie dento-faciale, ODF), or 2.3 per 100,000 residents, below the national 3.0. They average €899,611, +21 % on France and rank 21 of 70, after growth of +64 % since 2015, with 1,026 patients each. Younger specialists are almost missing: only 3 % are under 35. Stomatology, the medical specialty of the mouth and jaw, is down to 10 private practitioners from 15 five years earlier, 70 % of them 60 or over; their average fees are withheld under statistical confidentiality.
