Loire-Atlantique: plenty of patients, modest fees per patient

The Nantes paradox fits in two figures. A general dentist in Loire-Atlantique sees 975 different patients a year, well above the French 837. Yet average fees per full-time active practitioner come to €275,465, −9 % on the national mark, for a rank of 87th of 101. Per patient, then, billing is noticeably lower than elsewhere, which may reflect a case mix weighted towards routine care rather than prosthetics.

The split bears this out: €159,932 at the agreed tariff and just €115,533 in dépassements, the fees dentists set above that tariff, a smaller free-fee share than in most of western France. For a practice in Nantes, Saint-Nazaire or on the Guérande peninsula, comparing total fees with the national average misses the point: the patients are there, and the gap lies in the value of each treatment plan. The turnover per chair tool works on exactly that ratio.

The only Pays de la Loire département below the French average

All four other départements in the region post higher averages, from Maine-et-Loire to Mayenne, and the gap with the regional mean is −13 %. Density accounts for part of it: 55 general dentists per 100,000 residents, exactly the French 55, while the Sarthe and Mayenne have far fewer. Mixed practice is also rare, at 2 % against 6 % in France, so the gap does not come from dentists splitting their week with a salaried job.

For a département with a dental school in Nantes, headcount has barely moved: 814 at the end of 2020, 837 at the end of 2025. Young dentists are not missing (28 % under 35, 13 % aged 60 or more), and the average rose by +33 % from 2015 to 2025. Women make up 56 % of the profession, more than in Maine-et-Loire, Vendée or the Sarthe.

Specialists: ageing orthodontists, plenty of stomatologists

Orthodontics has the oldest profile in the département. Of 52 private orthodontists, 35 % are 60 or over and 4 % under 35, and there were 58 five years ago. Their €711,990 average stands −4 % against French orthodontists and −8 % against the regional figure, with 1,310 patients each. Practice sales in the specialty are likely to pick up in the coming years. Stomatology, the medical specialty of the mouth and jaw, is well established: 27 practitioners, one of the highest densities in France, 30 % in mixed private and salaried work, and an average of €420,978.
